Resumo(s): | O movimento feminista, embora não possa ser compreendido de modo uniforme,
representou um importante desafio para a sociedade e para a ciência em particular. Na sua luta
política pela igualdade, foi responsável pela denúncia da violência vivida por mulheres no
contexto de seus relacionamentos íntimos, conduzindo a integração da temática no campo dos
direitos humanos. Mais contemporaneamente, o campo da saúde pública tem vindo a
demonstrar atenção para este problema ainda oculto pelas normas sociais que privilegiam a
privacidade familiar e que colocam os homens em um estatuto de poder superior sobre as
mulheres. As repercussões sobre a saúde física e psicológica das vítimas de maus-tratos têm
sido amplamente documentadas em estudos, relatórios, tratados e convenções. As iniciativas
de prevenção da violência, ao promoverem a desocultação do fenómeno, têm sido hábeis em
aumentar a consciência social sobre o problema e propiciado um aumento no número de casos
registados nas entidades policiais, judiciais e instituições de apoio, representando, mais do
que um aumento na incidência, uma maior visibilidade do fenómeno.
No entanto, muito ainda está por fazer. Tradicionalmente encarado de modo
polarizado como um problema de origens socioculturais (consequência do machismo ) ou
como uma perturbação individual (como uma patologia, um desvio), o desafio está na
integração dos factores socioculturais ao atendimento prestado pelo sector saúde às vítimas de
violência conjugal. A saúde, devido à predominância do modelo biomédico, tem apresentado
dificuldades em trabalhar com os determinantes sociais , embora já os considere. Em muitos
casos, a integração destes aspectos está mais no nível teórico do que das práticas e, ainda
assim, com muitas imprecisões e distorções, tal como demonstra o equivocado uso do termo
género como sinónimo de sexo em diversas publicações e na formulação de políticas públicas.
Os cuidados de saúde primários, por definição, trabalham com uma perspectiva biopsico-
social e, deste modo, devem estar preparados para lidar com problemas que vão além da
enfermidade orgânica, tal como a violência de género. Neste aspecto, os médicos de família
actuantes nos cuidados de saúde primários são importantes actores para despiste e
acompanhamento destes casos devido ao contacto directo com todos os membros da família, a
continuidade do atendimento prestado e a relação de confiança que estabelecem com seus(uas) utentes. Desta forma, é urgente reflectir sobre o cuidado que tem sido prestado às
mulheres vítimas de maus-tratos, em especial no domínio dos cuidados de saúde primários.
Inserida no paradigma crítico oferecido pelo construcionismo social, análise de
discurso e feminismo, a presente investigação tem como objectivo mapear os discursos
circulantes nos cuidados de saúde primários sobre a violência de género e examinar suas
consequências para as práticas de cuidado oferecidas às mulheres submetidas à violência
conjugal. De modo subjacente, pretende desconstruir estes discursos a fim de construir
práticas de cuidado mais sensíveis às especificidades femininas, promovendo um saber mais
responsável com a melhoria das condições de vida das mulheres - mas também dos homens,
entendendo o género como algo que se faz nas relações interpessoais e reconhecendo que os
padrões da masculinidade hegemónica também podem ser opressivos para os homens.
Foram realizados três estudos, os quais procuraram mapear as diferentes
manifestações do discurso sobre a violência de género. Esta estratégia, longe de buscar a
generalização dos dados ou almejar revelar a verdade sobre o fenómeno, relaciona-se à
compreensão dos saberes como parciais, limitados e historicamente situados. No Estudo I
(Investigações sobre mulheres e violência em Portugal), fazemos um levantamento dos
trabalhos de mestrado e doutoramento realizados nas universidades portuguesas sobre
mulheres e violência. Os resultados indicam que ainda este é um tema recente e em expansão.
No estudo II (Revistas médicas: a invisibilidade da violência de género em publicações
nacionais), procedemos à análise crítica do discurso veiculado na Revista Portuguesa de
Clínica Geral e na Revista Portuguesa de Saúde Pública, concluindo-se que a violência
doméstica contra as mulheres não tem sido consistentemente discutida, pensada e pesquisada
no campo da saúde portuguesa, tendo como efeitos a invisibilidade de um grave problema de
saúde pública e a adopção de práticas individualizantes e pouco efectivas. No Estudo III
(Discurso de Médicos de Família actuantes nos Cuidados de Saúde Primários), foram
entrevistados 11 profissionais actuantes no município de Braga. Os resultados confirmam os
achados dos estudos anteriores, demonstrando a necessidade de incorporar novos saberes às
práticas destes profissionais. The feminist movement, though it could not be understood in a uniform way, represented an important challenge for the society and in particular, for the science. In its political struggle for the equality, it was responsible for the denunciation of the violence experienced by women in the context of their intimate relationships, leading the integration of the theme in the humans rights field. More contemporarily, the field of the public health has come to demonstrate attention for this problem, which is still hidden by the social standards that privilege the familiar privacy and that place men in a position of superior power compared to women. The repercussions on the physical and psychological health of the victims of intimate partner violence have been widely registered in studies, reports, treaties and conventions. The initiatives of violence prevention, more than bringing this issue to light, have been able to increase the social conscience on the problem and propitiated an increase in the number of cases registered in the police, judicial entities and institutions of support, representing, more than an increase in the incidence, a bigger visibility of the phenomenon. However, there is still a lot to be done. Traditionally faced in way, that is, polarized as a problem of socio-cultural origins (consequence of the "male chauvinism") or as an individual disorder (like a pathology, a diversion), the challenge is to integrate socio-cultural factors in the health sector service to the victims of conjugal violence. The health sector, due to the predominance of the biomedical model, has been facing difficulties in working with social determinants , though it already considers them. In many cases, the integration of these aspects is more in the theoretical level rather than practical one and, even so, with many distortion and inaccuracies, such as demonstrated to the misunderstood use of the term gender as a synonym of sex in several publications and in the formularization of public politics. The primary health care, for definition, works with a bio-psycho-social perspective and, in this way, must be prepared to deal with problems that go beyond the organic disease, such as gender-based violence. In this aspect, family physicians are important characters for mislead and accompaniment of these cases due to the direct contact with all the members of the family, attendance continuity and trust relationships with their clients.Thus, it is urgent to reflect on service that is provided to battered women, especially in the primary health care. Inserted in the critical paradigm offered by the social constructionism, discourse analysis and feminism, the present inquiry aims to map the circulating discourses in the primary health care on the gender-based violence and to examine its consequences for the practices of care offered to women victim of conjugal violence. In an underlying way, it also intends to deconstruct these discourses in order to build practices of care more sensible to female specificities, promoting a more responsible knowledge with the improvement of the conditions of life of women - but also of men, understanding gender as something that is constructed in the interpersonal relations and recognizing that hegemonic masculinity can also be oppressive for men. Three studies had been carried out, which looked at "mapping" the different manifestations of the discourse on gender-based violence. Far from looking for the generalization of data or longing to reveal the truth on the phenomenon, this strategy is linked to the understanding of knowledge as partial, limited and historically situated. In Study I (Investigations on women and violence in Portugal), we made a research of the fulfilled works of master’s degree and post-graduation (PhD) courses carried through in the Portuguese universities on women and violence field. The results indicate that this is still a recent subject and in expansion. In Study II (Medical Journals: the invisibility of gender-based violence in national publications), we proceed to a critical analysis of discourse in Portuguese Journal of General Practice (Revista Portuguesa de Clínica Geral) and Portuguese Public Health´s Journal (Revista Portuguesa de Saúde Pública), concluding that the domestic violence against women has not been consistently discussed, thought and investigated in the field of the Portuguese health, taking as effects the invisibility of a serious problem of public health and the adoption of individualist and poorly effective practices. In Study III (Discourse of family practice physicians that act in the Primary Health Care), 11 professionals that act in the city of Braga were interviewed. The results confirm the findings of the previous studies, demonstrating the necessity of incorporating new knowledge to the practices of these professionals. |
