Resumo(s): | A colaboração terapêutica desenvolvida ao longo do processo terapêutico entre cliente e
terapeuta, enquanto dimensão nuclear da aliança terapêutica, tem sido apontada como uma das
principais contribuições para a mudança terapêutica. Tendo em consideração a importância da
colaboração terapêutica, E. Ribeiro, A. P. Ribeiro, Gonçalves, Horvath e Stiles (2012) desenvolveram
o Sistema de Codificação da Colaboração Terapêutica (SCCT), com o intuito de analisar a
colaboração terapêutica a um nível microanalítico. O SCCT baseia-se num modelo desenvolvimental
da colaboração terapêutica, em que as intervenções do terapeuta e as respostas do cliente são
consideradas de acordo com uma Zona de Desenvolvimento Proximal Terapêutica (ZDPT), definida
como a distância entre o nível atual e o nível potencial do desenvolvimento do cliente, e na qual a
mudança do cliente parece ocorrer.
O principal objetivo deste trabalho foi explorar os processos interativos subjacentes ao
desenvolvimento da colaboração terapêutica num caso de sucesso seguido em Terapia Centrada no
Cliente. A díade participante neste estudo de caso foi constituída por uma terapeuta treinada em
Terapia Centrada no Cliente e uma cliente com depressão. O SCCT foi usado com o intuito de
caracterizar os processos interativos subjacentes à colaboração terapêutica desenvolvida ao longo do
processo terapêutico deste caso.
Os resultados indicam que ao longo do processo terapêutico, a díade trabalhou quase sempre
dentro da ZDPT, i.e. grande parte do tempo terapeuta e cliente interagiram de forma colaborativa.
Além disso, a terapeuta privilegiou as intervenções de suporte no problema da cliente, intervenções
estas que a cliente tendia a validar, indicando uma experiencia de segurança. Nos escassos momentos
em que a cliente invalidou a intervenção do terapeuta, manifestando uma experiência de risco
intolerável, a colaboração terapêutica foi restabelecida por um movimento de recuo da terapeuta com
vista ao reencontro com a cliente dentro da ZDPT, iniciando a restauração da colaboração terapêutica.
De salientar que as intervenções da terapeuta mais frequentes foram de suporte, permitindo elaborar a
ideia de equilíbrio entre intervenções de suporte e de desafio para a mudança, um dos pressupostos
centrais do modelo subjacente ao SCCT.
Nesta dissertação, os resultados são discutidos no contexto deste caso clínico e da literatura
teórica e empírica, discutindo-se também as implicações do resultados para o desenvolvimento da
relação entre colaboração e mudança em Terapia Centrada no Cliente. Therapeutic collaboration developed through the therapeutic process between client and therapist, while nuclear dimension of the therapeutic alliance, has been identified as one of the main contributions to therapeutic change. Given the importance of therapeutic collaboration, E. Ribeiro, A. P. Ribeiro, Gonçalves, Horvath and Stiles (2012) developed the Therapeutic Collaboration Coding System (TCCS), in order to analyze the therapeutic collaboration at a microanalitic level. The TCCS is based on a developmental model of therapeutic collaboration, which considers the coordination of the therapist interventions and the clients’ responses according to a Therapeutic Zone of Proximal Development (TZPD), defined by the distance between a actual level and the potential level of the client development, and where the client’s change should to occur. The main goal of this study was to explore the interactive processes underlying the development of a therapeutic collaborative in a good outcome case Client Centered Therapy. The dyad participant in this case study included a therapist trained in Client Centered Therapy and a depressive client.. The TCCS was used in order to characterize the interactive processes underlying the therapeutic collaboration developed throughout the therapeutic process in this case. The results indicate that throughout the therapeutic process, the dyad worked mostly within the TZPD, i.e. most of the time therapist and client interacted in a collaborative mode. Furthermore, the therapist interventions were mostly focused on supporting the client’s problem, usually the client validated these interventions, indicating a safety experience. In the few moments when the client invalidated the therapist's interventions, expressing an experience of intolerable risk, the therapist moved backward to a more safety zone of the client’s TZPD, initiating the restoring of the therapeutic collaboration. Since the supporting problem was the most frequent therapist intervention, this result allows to elaborating the idea of balance between supporting and challenging therapist’s interventions, one of the main assumptions of the model underlying the TCCS. In this dissertation, the results are discussed in the context of this case study and theoretical and empirical literature, discussing also the implications of the results for the development of the relationship between collaboration and change in Client Centered Therapy. |
