Resumo(s): | Uma lente de contato sofre durante o seu período de utilização várias alterações que
podem comprometer o sucesso de uma boa adaptação, ou pelo menos, de uma adaptação que
seja plenamente confortável e que proporcione uma visão adequada para o paciente. São vários
os fatores que podem ter essa influência. A superfície de uma lente de contato (LC) é suscetível
a que ocorram várias dessas alterações e torna-se importante a sua análise detalhada para
perceber melhor os mecanismos de afetação que possam ocorrer. Avaliar essas alterações
permite entender melhor os eventuais processos de deterioração da LC durante o período de
utilização. O objetivo deste trabalho consiste na avaliação da qualidade da superfície de lentes de
contacto “in vivo” mediante a medida da topografia corneal sobre a superfície da LC. Os
parâmetros de controlo foram o índice de regularidade da superficie (SRI), o índice de assimetria
da superfície (SAI), o índice I-S, a excentricidade (e), o raio apical, os raios queratométricos e a
análise das aberrações da superfície através dos polinómios de Zernike. Esta análise foi realizada
ao longo de um mês de utilização das LC e foram realizadas topografias no 1º dia, no 15º dia e
no 30º dia de uso da mesma lente de contacto de silicone hidrogel (Air Optix Aqua, Ciba Vision)
combinada com o mesmo produto de manutenção (SoloCare Aqua, Ciba Vision). Nos dias em
que foram realizadas as topografias, foi feita uma avaliação na parte da manhã e outra ao fim do
dia, em ambos os olhos de 18 pacientes. O objetivo foi tentar analisar eventuais alterações na
qualidade da superfície de refração da LC ao longo do dia e do mês de utilização e por
consequência na qualidade ótica do olho. Foi realizada também uma análise a estes parâmetros
após a aplicação de uma gota de lágrima artificial na LC para verificar o seu potencial na
normalização destes mesmos parâmetros.
A análise topográfica mostrou ser uma ferramenta útil para determinar a evolução temporal da
qualidade ótica da superfície das LC. Em geral, observou-se uma flutuação dos parâmetros
topográficos e da qualidade ótica da superfície da lente ao longo do mês de uso das lentes com
uma redução por volta dos 15 dias e um novo aumento ao final do mês de utilização. A
aplicação de lágrima artificial como estratégia para regularizar a superfície da LC mostrou-se útil
para a avaliação das aberrações de tipo esférico, no entanto mostrou que pode ter efeitos
negativos no incremento das aberrações de tipo comático bem como nos parâmetros
topográficos de assimetria da superfície. Um dos parâmetros que sofreu mais alterações com a
aplicação da lágrima artificial foi o parâmetro de excentricidade no meridiano mais curvo (esteep). A contact lens undergoes during its period of use several changes that can compromise the success of a good fit, or at least an adaptation to be fully comfortable and providing good vision for the patient. There are several factors that may influence this. The surface of a contact lens (CL) is likely to undergo these changes and it becomes important a detailed analysis to better understand the possible degradation processes of the CL surface during the period of use. The aim of this study was to assess the quality of the surface of a CL “in vivo” by measuring the corneal topography on the surface of the CL. The control parameters wear the surface regularity index (SRI), the surface asymmetry index (SAI), the I-S index, the eccentricity (e), the apical radius, the keratometry and the analysis of the surface aberration through the Zernike polynomials. This analysis was performed over a month of use of the same silicone hydrogel CL (Air Optix Aqua, Ciba Vision) combined with the same care solution (SoloCare Aqua, Ciba Vision) and topographies are being carried out on day 1, on the 15th and 30th days of use. On days that are held topographies an assessment is made in the morning and another in the evening, in both eyes of 18 patients. The aim was to try to analyze possible changes in the quality of the front surface of the CL throughout the day and month of wear therefore the quality optics of the eye. There will be also an analysis of these parameters after applying a drop of artificial tears in the CL to check the potential for normalization of these parameters. The topographical analysis showed to be a useful tool to determinate the temporal evolution of the optical quality of the surface of CL. In general, there was a fluctuation in the topographic parameters and the optical quality of the lens surface throughout the month of lens wear with a reduction of around the 15 day and a further increase at the end of the month. The application of artificial tears as a strategy to regulate the surface of the CL proved to be useful for the evaluation of the aberrations of spherical type, showed however that may have negative effects on the increase in coma type aberrations as well as in the topographic parameters of asymmetry of the surface. One of the parameters that has changed more in the application of artificial tears is the eccentricity parameter in curver meridian (esteep). |
