Resumo(s): | O desenvolvimento de metodologias moleculares mais sensíveis e automatizadas que fornecem
resultados rápidos e precisos representa um grande progresso no diagnóstico de infeções fúngicas
invasivas. A rápida deteção da presença de um agente patogénico é essencial para melhorar o desfecho
clínico dos pacientes. Testes panfúngicos têm potencial para detetar vários fungos patogénicos, comuns
ou emergentes. No entanto, é necessário aplicar técnicas adicionais para realizar identificação ao nível
da espécie, estabelecer o plano terapêutico mais adequado e aumentar a sobrevivência dos pacientes.
As infeções causadas por Candida, também conhecidas como Candidíases, são uma das
infeções nosocomiais mais comuns causadas por fungos, sendo C. albicans a mais bem caracterizada.
No entanto, espécies não-albicans, como C. glabrata, C. krusei, C. parapsilosis, C. tropicalis e C. auris,
são consideradas uma ameaça global à saúde pública. C. auris é um fungo emergente multirresistente,
responsável por vários surtos em todo o mundo, sendo descrito como particularmente perigoso em
ambientes hospitalares. Além disso, os métodos laboratoriais convencionais falham frequentemente em
identificar corretamente C. auris.
Os principais objetivos desta tese foram incluir na metodologia de PCR em multiplex
desenvolvida anteriormente, marcadores específicos para deteção de C. auris e um marcador
panfúngico. Bem como avaliar a possibilidade de converter o painel multiplex “CandidaPanel” analisado
por GeneScan em PCR quantitativo em tempo real. Neste estudo, foi possível incluir o marcador
específico de C. auris e a metodologia atualizada foi adequada para a deteção e identificação específica
de C. auris, mantendo a especificidade na identificação das restantes espécies do painel em multiplex.
No entanto, o marcador panfúngico revelou-se incompatível com o marcador de C. auris, não sendo
possível a sua inclusão no painel PCR em multiplex. Este painel também demonstrou capacidade de
identificar diferentes espécies de Candida em infeções mistas simuladas e ainda de identificar
corretamente C. auris, diretamente de colónias, por colony-PCR. Uma vez que os produtos de PCR
obtidos para as espécies de Candida apresentaram temperaturas de fusão muito semelhantes, a
conversão da metodologia multiplex para PCR quantitativo em tempo real baseada apenas em SYBR
Green não foi possível.
Este trabalho permitiu a inclusão do marcador de C. auris, no painel multiplex “CandidaPanel”,
permitindo a identificação específica desta espécie multirresistente e auxiliando o seu diagnóstico. The development of more sensitive and automated molecular methodologies that deliver fast and accurate results represents a huge improvement in the diagnosis of invasive fungal infections. Timely detection of the presence of a fungal pathogen is very important to improve the clinical outcome of the patients. Panfungal PCR diagnostic assays can potentially detect a broad range of fungal pathogens, common or emerging. However, additional techniques to perform identification at the species level are crucial to establish the most appropriate therapeutic approach and increase the patients’ survival chances. Candida infections, also known as Candidiasis, are considered to be one of the most common nosocomial infections caused by fungi, being C. albicans the most well-characterized species. However non-albicans Candida species, such as C. glabrata, C. krusei, C. parapsilosis, C. tropicalis and C. auris, have been considered a significant global health threat. C. auris is a multidrug-resistant emerging fungus responsible for several outbreaks worldwide, being particularly dangerous in hospital environments. Additionally, standard laboratory methods every so often fail to properly identify C. auris, being frequently misidentified. The main goals of this thesis project were to include C. auris specific markers and a panfungal marker in the previously developed multiplex PCR methodology. As well as evaluating the possibility of converting the developed multiplex CandidaPanel analysed by GeneScan to quantitative real-time PCR. In this project, it was achieved the inclusion of the C. auris specific marker and the updated multiplex methodology was suitable for the detection and specific identification of C. auris, maintaining the specificity for the remaining species in the multiplex panel. However, the panfungal marker showed to be incompatible with the C. auris specific marker, and its inclusion in the multiplex PCR panel was not possible. This panel also demonstrated the ability to identify different Candida species in simulated mixed infections and to perform accurate identification directly from fungal colonies, by colony-PCR. The conversion of the CandidaPanel multiplex methodology to quantitative real-time PCR was not possible since the PCR products obtained for the Candida species had very similar melting temperatures, hindering the Candida species differentiation directly using SYBR Green. This work allowed the inclusion of a specific marker for C. auris in the multiplex panel CandidaPanel, allowing the specific identification of this multi-resistant species and aiding its diagnosis. |
