Implementação da ferramenta Failure Mode and Effects Analysis na avaliação e gestão do risco numa empresa de construção civil

Abstract

A presente dissertação foi realizada no âmbito do termo do Mestrado Integrado em Engenharia e Gestão Industrial e teve como principal objetivo implementar a ferramenta FMEA para a avaliação e gestão do risco na empresa DST, S.A. Para dar resposta aos requisitos da norma ISO 9001, a DST, fazia a identificação dos riscos e oportunidades, no entanto não havia qualquer tratamento de priorização dos riscos identificados. Numa perspetiva de melhoria contínua e face às exigências do mercado, a revisão da norma ISO 9001 sugere a adoção de uma abordagem sistemática e contínua na consideração dos riscos em todo os Sistema de Gestão da Qualidade. O trabalho foi incidido na identificação dos riscos existentes associados aos vários processos do Sistema. Na prática, a avaliação e gestão do risco iniciou-se pela elaboração da análise SWOT por processo, por forma a detalhar as diferentes dimensões da SWOT. De seguida, utilizando uma dinâmica brainstorming, num grupo multidisciplinar, identificaram-se os riscos. Através da ferramenta FMEA procedeu-se à análise e avaliação dos riscos identificados. Começou-se por identificar os efeitos dos riscos e classificá-los quanto ao seu grau de severidade. Na sequência, foram identificadas causas e atribuído um valor de índice de ocorrência a cada causa. Após serem identificados os controlos existentes são dados valores de deteção. Posto isto, foi calculado o RPN de forma automática e obtida uma classificação desse valor quanto à sua tipologia de risco – baixo, médio ou alto. Através da aplicação desta metodologia de avaliação e gestão do risco a um departamento da organização, foram identificados oito riscos sendo um deles comum aos três processos deste departamento. Da FMEA deste departamento, resultaram trinta RPNs, em que onze são classificados como baixo, catorze como médio e oito como alto. Para os riscos com uma classificação “baixo” foram definidas ações de monitorização, para os restantes foram definidas ações para serem implementadas a médio e longo prazo. Com a implementação desta ferramenta, a empresa passou a agir de forma preventiva nos vários processos que englobam o Sistema de Gestão da Qualidade.

The present dissertation was carried out within the scope of the Master's Degree in Industrial Engineering and Management and had as main objective to implement the FMEA tool for the evaluation and risk management in the company DST, S.A. In order to meet the requirements of the ISO 9001 standard, DST identified risks and opportunities, but there was no prioritization treatment of identified risks. In a perspective of continuous improvement and in face with the market demands, the revision of the ISO 9001 standard suggests the adoption of a systematic and continuous approach of the risks throughout the Quality Management System. The work was focused on identifying the existing risks associated with the various processes of the system. In practice, risk assessment and risk management started with the development of the SWOT analysis by process, in order to detail the different dimensions of the SWOT. Then, using a dynamic brainstorming, in a multidisciplinary group, the risks were identified. Through the FMEA tool, the identified risks were analyzed and assessed. We began by identifying the effects of the risks and classifying them according to their degree of severity. Subsequently, causes were identified and an occurrence index value was assigned to each cause. After the existing controls are identified, detection values are given. Therefore, the RPN was calculated automatically and it was obtained a classification of this value as to its typology of risk - low, medium or high. Through the application of this methodology of risk assessment and management to a department of the organization, eight risks were identified and one of them is common to the three processes of this department. Based on the FMEA of this department, resulted thirty RPNs, in which eleven are classified as low, fourteen as average and eight as high. For the risks with a "low" rating, monitoring actions were defined and for the others actions were defined to be implemented in the medium and long term. With the implementation of this tool, the company started to act in a preventive way in the various processes that encompass the Quality Management System.
