Resumo(s): | Acute leukemia (AL) are hematologic malignancies that occur through the malign
transformation of stem cells or myeloid precursors (AML) or lymphoid precursors cells
(ALL), changing their differentiation and survival, leading to the accumulation of
immature cells, generically called blasts. These malignancies have aggressive clinical
courses and, if untreated, are rapidly fatal. The anti-leukemia therapy is based on
cytotoxic polichemotherapy associated to the proper supportive treatment. Although
conventional chemotherapy used results in good outcomes for young patients, its
application is limited in aged patients and the risk of disease relapse remains high.
Thus, the elucidation of the mechanism underlying leukemia cell’s resistance to
conventional chemotherapy is still a major challenge.
Mounting evidence has shed light on the importance of the proteolytic systems
(ubiquitin-proteasome system – UPS - and autophagy) in cancer cell biology, including
in hematological malignancies. Curiously, autophagy is found activated in response to
a variety of therapeutic drugs and most of the existing drugs designed to kill cancer
cells were shown to induce autophagy. Therefore, autophagy is emerging as a crucial
target to anticancer therapy. Recent evidences suggested that UPS was also
associated with a large number of cellular processes essential to tumorogenesis and
chemoresistance of cancer cells. Indeed, promising studies described that proteasome
inhibitors possess antitumor proprieties and, in vivo studies showed that these agents
induce tumor growth delay.
This work aimed to determine the role of proteolytic systems in AL cell’s response to
conventional chemotherapy. For such purpose, an acute lymphoid leukemia cell line
(Jurkat) and three acute myeloid leukemia cell lines (HL-60; KG-1; NB4) were used as
in vitro models. AL cells were treated with cytarabine and/or doxorubicin (drugs used
in conventional chemotherapy) and simultaneously the autophagy and/or UPS were
pharmacological inhibited (chloroquine, bafilomycin A1, 3-MA or bortezomib,
respectively). The coordinated regulation of the proteolytic systems and HSP90 (an
indirect modulator of proteolytic systems) was also assessed. Results obtained in this work suggested that the inhibition of proteolytic systems by
pharmacological manipulation increases the susceptibility of ALL and AML cells to
cytarabine and doxorubicin and thus might have a potential therapeutic value. The
data obtained on the transcriptional and non-transcriptional regulation of key
autophagic regulators and on the proteasome activity provided evidence for the
induction of both proteolytic systems by doxorubicin and cytarabine. Additionally,
when we accessed the effect of an inhibitor of HSP90 on autophagy and UPS activity
in AL cells, it was clear that 17-AAG promotes an increase of autophagy and a
decrease of UPS activity, in contrast to some evidence previously reported in cancer
studies.
The data from this work supports the relevance of considering the manipulation of
proteolytic systems as a fundamental parameter in AL therapy. As leucemias agudas (LA) são neoplasias hematológicas em que o processo transformador ocorreu em células estaminais ou percursores mielóides (LAM) ou em células percursoras linfóides (LAL), alterando a sua diferenciação e sobrevida, levando à acumulação de células imaturas, genericamente designadas de blastos. Estas doenças têm um curso clínico agressivo e, se não forem tratadas são rapidamente fatais. A terapêutica anti-leucémica baseia-se em poliquimioterapia citotóxica associada a tratamento de suporte adequado. Embora a quimioterapia convencional usada desempenhe bons resultados em pacientes jovens, a sua aplicação é limitada em pacientes idosos e o risco de recaída continua elevado. Desta forma, a elucidação dos mecanismos celulares subjacentes à resistência de células leucémicas à quimioterapia convencional constitui ainda um grande desafio. Inúmeras evidências demonstraram a importância dos sistemas proteolíticos (sistema ubiquitina-proteassoma, UPS, e autofagia) em células cancerígenas, incluindo em doenças malignas hematopoiéticas. Curiosamente, a autofagia encontra-se activada em resposta a vários fármacos e a maioria dos fármacos existentes, dirigidos à eliminação de células cancerígenas, induzem autofagia. Assim, este processo emerge como um ator crucial na terapia anticancerígena. Evidências recentes sugerem que o proteassoma também está associado a um grande número de processos celulares essenciais para a formação de tumores e para a quimioresistência das células malignas. De facto, estudos promissores descreveram que os inibidores do proteassoma possuem propriedades anti-tumorais e, estudos in vivo demonstraram que esses agentes inibem o crescimento tumoral. Este trabalho teve como objectivo principal determinar o papel dos sistemas proteolíticos na resposta de células de LA à quimioterapia convencional. Para isso, uma linha celular de leucemia linfoblástica aguda (Jurkat) e três linhas celulares de leucemia mieloblástica aguda (Hl-60, KG-1 e NB4) foram usadas como modelos in vitro. As células de LA foram tratadas com citarabina e doxorubicina (fármacos usados na quimioterapia convencional) e simultaneamente a autofagia e/ou o proteassoma foram farmacologicamente inibidos (cloroquina, bafilomicina A1, 3-MA e bortezomib, respetivamente). A regulação coordenada dos sistemas proteolíticos e o papel da HSP90 (regulador indirecto dos sistemas proteolíticos) foi também avaliada. Os resultados obtidos sugerem que a inibição dos sistemas proteolíticos pela sua manipulação farmacológica aumenta a susceptibilidade das células de LLA e LMA à citarabina e doxorubicina, podendo assim ser considerados potenciais alvos terapêuticos. Os dados obtidos na regulação transcricional e não transcricional dos reguladores autofágicos chave e na actividade do proteassoma demonstraram a indução de ambos os sistemas proteolíticos pela doxorubicina e pela citarabina. Adicionalmente, quando verificamos o efeito de um inibidor da HSP90 na actividade da autofagia e do proteassoma em células de LA, foi claro que o 17-AAG promove um aumento da autofagia e um decréscimo da actividade do proteassoma, contrariando algumas evidências anteriores. Os dados deste trabalho mostram a relevância de se considerar a manipulação dos sistemas proteolíticos como um parâmetro fundamental na terapia de LA. |
