Resumo(s)O objetivo deste estudo foi determinar a resposta da atividade eletrofisiológica, de diferentes regiões da retina, mediante eletrofisiologia multifocal durante o período de adaptação a lentes de contacto multifocais de desenho centro-perto e desenho centro-longe, numa lógica de adaptação contra lateral. Foram recrutados 4 voluntários présbitas que nunca tinham utilizado lentes de contacto multifocais. Realizaram-se as medidas de eletrofisiologia multifocal com lentes de contacto monofocais (determinação do valor baseline) e multifocais. Registaram-se também os seguintes parâmetros visuais: acuidade visual de alto e baixo contraste, aberrometria e a perceção subjetiva do tamanho do halo luminoso. Após 15 dias de uso das lentes de contacto multifocais, repetiram-se as medidas de eletrofisiologia multifocal e restantes exames. Na análise das regiões da retina por zonas anelares, as respostas médias no mfERG, em olhos adaptados com as lentes de contacto monofocais e com lentes de contacto multifocais de desenho centro-longe, tendem a apresentar comportamentos semelhantes nos dois dias avaliados (dia 1 e dia 15). Relativamente aos olhos adaptados com lentes de contacto multifocais de desenho centro-perto, observou-se um pequeno atraso no tempo implícito com a lente de contacto multifocal no dia um, que tende a diminuir após os 15 dias de adaptação à multifocalidade. Relativamente à avaliação das diferentes regiões da retina por quadrantes, nos olhos adaptados com lentes de contacto multifocais centro-longe, registaram-se valores de P1 e N1 mais elevados nos quadrantes 1 e 4. O mesmo comportamento foi observado nos olhos adaptados com lentes de contacto multifocais centro-perto, tendo sido ainda observados menores amplitudes e tempos implícitos mais elevados com as lentes de contacto multifocais, nos quadrantes 2 e 3. As lentes de contacto multifocais de desenho centro-perto aparentam produzir um maior impacto na alteração da resposta eletrofisiológica da retina a curto-prazo. O atraso no tempo implícito obtido com lente de contacto multifocal centro-perto no primeiro dia, com a posterior recuperação após o décimo quinto dia de uso, sugere uma adaptação da retina a este dispositivo multifocal.
The objective of this study was to determine the response of the electrophysiological activity of different regions of the retina by means of multifocal electrophysiology during the adaptation period to multifocal contact lenses with center drawing near and center drawing far, in a logic of lateral adaptation. Four presbyopic volunteers who had never used multifocal contact lenses were recruited. Measurements of multifocal electrophysiology with monofocal contact lenses (baseline value) and multifocal measurements were performed. The following visual parameters were also recorded: high and low contrast visual acuity; aberrometry and the subjective perception of the size of the luminous halo. After 15 days of multifocal contact lenses usage, multifocal electrophysiology and other examinations were repeated. In the analysis of the regions of the retina by ring zones, the mean responses in mfERG tend to present similar behaviors on the two days evaluated (day one and day fifteen), in eyes adapted with monofocal contact lenses and multifocal contact lenses with center-distance drawing. Regarding the center-near multifocal contact lenses-adapted eyes, there was a small delay in implicit time with the multifocal contact lens at day one, which tends to decrease after 15 days of adaptation to multifocality. Regarding the evaluation of the different regions of the retina by quadrants, in the eyes adapted with center-distance multifocal contact lenses, higher values of P1 and N1 were recorded in quadrants 1 and 4. The same behavior was observed in the eyes adapted with center-near multifocal contact lenses, and smaller amplitudes and implicit times were observed with multifocal contact lenses in quadrants 2 and 3. Multifocal contact lenses with center-near design appear to have a greater impact on the alteration of the electrophysiological retinal response in short-term. The delay in implicit time obtained with center-near multifocal contact lens on the first day, with subsequent recovery after the fifteenth day of use, suggests an adaptation of the retina to this multifocal device.
