Educar para a saúde na reabilitação da sexualidade de homens com lesão vertebro-medular : um estudo sobre as suas percepções e práticas durante o processo de reabilitação

Abstract

A educação assume uma função vital nas intervenções de reabilitação. Tratando-se a reabilitação sexual no utente com LVM de um processo complexo e moroso, nada é mais importante do que a perspectiva do utente para compreender quais são as necessidades em educação para a saúde neste âmbito. Assim, esta investigação teve como principais objectivos: identificar quais são as necessidades em reabilitação dos homens após LVM e quais são as suas percepções sobre a abordagem educativa feita pelos profissionais de saúde em contexto hospitalar. Com essa intenção realizou-se um estudo de natureza qualitativa, com recurso a uma entrevista semi-estruturada, a realizar a numa amostra de conveniência de homens com LVM de origem traumática (N=12) que estiveram internados num serviço de reabilitação. Todos os homens entrevistados com LVM, como consequência de um acidente, confrontaram-se com uma situação nova com várias limitações a nível físico, psicológico e social. Estes homens demonstraram que tinham comportamentos sexuais diferentes que exigiam diferentes formas de ajuda em educação sexual, mas, em sua opinião, a abordagem da sexualidade pelos profissionais de saúde apresentou lacunas nos conhecimentos e experiência com utentes com alterações da sexualidade, informando-os pouco sobre as inúmeras dificuldades que experienciavam para se adaptarem à nova condição física, imagem corporal e auto-estima. Os utentes com LVM procuraram ter algum apoio e aconselhamento sexual ao colocarem questões sobre as preocupações e problemas sexuais. Quando ficaram com a incapacidade física, todos consideraram que a sua dimensão sexual também tinha sofrido alterações e expressaram desconhecimento sobre como poderia a função sexual ser prejudicada, nomeadamente: a erecção, o orgasmo e ejaculação. Destes resultados emerge a necessidade de investir na formação dos profissionais de saúde, assim como na concepção e implementação de programas de educação sexual na reabilitação do utente com LVM.

Education assumes a vital role in rehabilitation interventions. Because sexual rehabilitation in patients with spinal cord injury is a complex and a long-term process, nothing is more important than the patients’ perspectives to better understand what their needs in health education in this area are. In this sense, this research had the following main objectives: to identify the needs in the rehabilitation of men after spinal cord injuries; and what their perceptions about the educational approach made by health professionals in hospital settings are. With this purpose in mind, a qualitative study, using a semi-structured interview to be applied in a convenience sample of men with traumatic spinal cord injury (N=12), who were hospitalized in a rehabilitation service was carried out. All the men with spinal cord injuries interviewed, as a consequence of an accident, faced a new situation with serious physical, psychological and social limitations. These men demonstrated they had different sexual behaviours requiring different forms of aids in sexual education, but in their opinion, the approach to sexuality by health professionals showed gaps in knowledge and experience with patients with alterations in sexuality, informing them little about the many difficulties they would experience in having to adapt to a new physical condition, body image and self-esteem. The patients with spinal cord injury sought out some sexual support and advice, by asking questions regarding their sexual concerns and sexual problems. When they became physically disabled, they all felt that their sexual dimensions had also changed, and expressed ignorance about how their sexual performance would eventually be impaired namely: obtaining an erection, orgasm and ejaculation. From these results, emerges the need to invest in the training of health professionals, as well as the design and implementation of sexual education programs in the rehabilitation of patients with spinal cord injuries.
