O papel da vinculação no ajustamento conjugal em mulheres com HPV

Abstract

O impacto de ter uma doença crónica, particularmente o diagnóstico da infeção por HPV, gera consequências físicas, psicológicas, conjugais, familiares e sociais. A presença do HPV na vida da mulher, e sua associação a verrugas e lesões genitais diminui a satisfação sexual. Neste contexto, uma vinculação segura relaciona-se com menor stresse provocada pela doença e promove uma melhor adaptação emocional e ajustamento conjugal. OBJETIVO: Avaliar se a vinculação mediava a relação entre variáveis sociodemográficas e clínicas com as variáveis satisfação sexual e ajustamento conjugal, em mulheres com HPV. MÉTODO: Realizou-se uma path analysis com bootstrapping (IBM SPSS AMOS versão 24.0), com uma amostra de 209 mulheres com HPV. Tendo em conta o modelo inicial, as vias foramreduzidas sequencialmente com base na sua significância e contribuição para o ajustamento geral do modelo. RESULTADOS: Verificou-se que a vinculação mediou a relação entre a idade e o ajustamento conjugal; entre o tipo de HPV e o ajustamento conjugal e parcialmente entre a satisfação sexual e o ajustamento conjugal. CONCLUSÃO: De acordo com os resultados, seria relevante intervir nas mulheres portadoras do HPV ao nível da vinculação e insatisfação sexual, com programas de intervenção diferenciados tendo em consideração a idade e o tipo do HPV (baixo/alto risco).

The impact of having a chronic illness, particularly the diagnosis of HPV infection, results in physical, psychological, marital, family, and social consequences. The presence of HPV in a woman’s life, and its association with warts and genital lesions, decreases sexual satisfaction. In this context, a safe bonding favors the reduction of stress caused by the disease and contributes to a better emotional adaptation and marital adjustment. OBJECTIVE: To assess whether attachment mediated the relationship between sociodemographic, clinical, sexual satisfactionand marital adjustment variables in women with HPV. METHOD: A path analysis was performed with bootstrapping (IBM SPSS AMOS version 24.0), with a sample of 209 women with HPV. Taking into account the initial model, the pathways were reduced sequentially based on their significance and contribution to the overall fit of the model. RESULTS: Attachment mediated the relationship between age and marital adjustment; between the type of HPV and marital adjustment and partially between sexual satisfaction and marital adjustment. CONCLUSIONS: According to the results, it would be relevant to intervene in women with HPV regarding the level of sexual attachment and dissatisfaction, with differentiated intervention programs taking into account the age and type of HPV (low / high risk).
