Estudo das necessidades de médicos especialistas na Região Norte de Portugal

Abstract

Ao longo dos anos tem-se verificado a nível mundial uma crise no setor da saúde, que é caraterizada pela escassez e má distribuição dos recursos humanos em saúde, dentro e entre países. Na literatura existente são identificados vários fatores que influenciam esta crise no setor da saúde, como por exemplo, a migração dos profissionais de saúde ou as más condições de trabalho que encontram. Cabe aos decisores dos recursos humanos em saúde ter em conta estes condicionalismos e determinar/projetar o número e o tipo de profissionais de saúde necessários para assegurar os serviços de saúde. Vários modelos de planeamento foram desenvolvidos e aplicados, sendo os modelos baseados na oferta, procura, necessidade e "benchmarking" os principais modelos de projeções encontrados na literatura. Portugal é um dos países com dificuldades na gestão de profissionais médicos, com situações de algum desequilíbrio de médicos disponíveis entre zonas rurais e urbanas. Na região norte, cabe a ARS Norte garantir o acesso aos cuidados de saúde, pela gestão dos seus recursos humanos e de infraestruturas como os hospitais ou centros de saúde. Um dos principais desafios prende-se com a formação em Internato Médico, nomeadamente ao nível da caracterização dos médicos existentes nas diferentes especialidades e nos diferentes hospitais e na sua articulação com a abertura de vagas para novos especialistas. Acresce ainda o facto dos recém formados poderem em termos de vínculo profissional transitar para outras ARS que não a ARS Norte, o que dificulta a gestão da oferta de médicos especialistas da ARS Norte. Nesse sentido, e em colaboração com a ARS Norte, foi realizado um estudo das necessidades de médicos da região Norte. Com base nos dados disponibilizados pela ARS Norte, relativos aos médicos em exercício nos hospitais e unidades locais de saúde na região norte, foi possível identificar as especialidades com insuficiência ou má distribuição de médicos especialistas, bem como os hospitais com necessidades específicas de algumas especialidades. Foi ainda definido um modelo de projeção da oferta até 2025 da evolução do número de médicos especialistas.

Over the last years there is a worldwide rising crisis in the health sector, which is characterized by scarcity and inefficient distribution of health human resources, within and between countries. In the literature several factors are identified as influencing the crisis in the health sector, such as the migration of health professionals or poor working conditions. Thus, the planners of human health resources should take into account these constraints and determine/project the number and type of health professionals needed to ensure health services. Several planning models have been developed and applied, but the models based on supply, demand, need and “benchmarking” appear to be the main models of projections in the literature. Portugal is one of the countries with several difficulties in the management of medical professionals, with situations of some imbalance of available doctors between urban and rural areas. In the northern region, ARS Norte should ensure access to health care, the management of its human resources and infrastructure such as hospitals or health centres. One of the major challenges is related to the training in medical internship, namely in terms of characterization of existing doctors in different specialties and different hospitals and in their relationship with the opening of new jobs for new specialists. Moreover the fact that recent graduates may be carried over in terms of employment status to other ARS than the ARS Norte, makes it difficult to supply management of medical experts of the ARS Norte. In this sense, a study of the medical needs of the northern region was conducted in collaboration with ARS Norte. Based on data provided by the ARS Norte, on practicing physicians in hospitals and local health units in the northern region it was possible to identify the specialties with insufficient or poor distribution of medical specialists and hospitals with specific needs of some specialties. It was also defined a supply projection model of the evolution of the number of specialist doctors until 2025.
