dc.description.abstractAo conceito de aliança terapêutica está inerente uma dimensão colaborativa que diz respeito ao envolvimento colaborativo entre cliente e terapeuta e cuja finalidade é promover uma mudança favorável do cliente (Horvath, 2006). No contexto da investigação que procura compreender os microprocessos envolvidos no desenvolvimento da aliança terapêutica, Ribeiro, E., Ribeiro, A.P., Gonçalves, Horvath e Stiles (2012) desenvolveram um modelo conceptual sobre o desenvolvimento da colaboração terapêutica, tomando como referencia o conceito de Zona de Desenvolvimento Proximal. Com base neste modelo e com o objetivo de analisar a colaboração terapêutica momento a momento, os autores desenvolveram o Therapeutic Collaboration Coding System – TCCS (Ribeiro et al., 2012). O principal objetivo da presente dissertação foi comparar o desenvolvimento da colaboração terapêutica em dois casos clínicos, um de sucesso e outro de insucesso terapêutico, seguidos numa modalidade individual de terapia narrativa desenvolvida a partir do modelo de reautoria de White e Epston (1990). O caso de insucesso decorreu ao longo de 20 sessões e o caso de sucesso ocorreu num total de 16 sessões. As sessões de ambos os casos foram codificadas tendo por base o TCCS. Os resultados relativos à evolução da colaboração terapêutica sugerem que, em ambos os casos, o terapeuta utiliza mais intervenções de desafio. No entanto, enquanto que no caso de insucesso o terapeuta suporta mais vezes a experiência problemática, no caso de sucesso o terapeuta suporta mais vezes a experiência de inovação da cliente. No que se refere à resposta das clientes, ambas tenderam a privilegiar respostas que indicavam experiências de segurança. Por outro lado, a cliente do caso de insucesso apresentouse mais oscilante entre o risco tolerável e o risco intolerável durante todo o processo, e a cliente do caso de sucesso mostrou uma evolução nas suas respostas mostrando um decréscimo da experiência de risco intolerável e aumento da resposta de risco tolerável ao longo da terapia. A análise efetuada com o State Space Grids (Lamey, Lewis, & Granic, 2004) sugeriu que ambas as díades do caso de sucesso e insucesso estabilizaram a maior parte das suas interações terapêuticas em episódios cuja resposta da cliente indicava segurança. Estes episódios, por sua vez, incluíram os diferentes tipos de intervenção do terapeuta. Os resultados irão ser discutidos tendo em conta a literatura existente neste domínio de investigação e a sua contribuição para compreender a colaboração terapêutica em casos de sucesso e de insucesso terapêutico, procurando perceber a influência das interações terapêuticas na ocorrência de mudança nestes processos e levantando hipóteses que poderão ser analisadas em amostras maiores.por
dc.description.abstractThe concept of therapeutic alliance has inherently collaborative dimension with respect to collaborative involvement between client and therapist and whose purpose is to promote a favorable change in the client (Horvath, 2006). In the context of research that seeks to understand the micro processes involved in the development of therapeutic alliance, Ribeiro, E., Ribeiro, A.P., Gonçalves, Horvath e Stiles (2012) developed a conceptual model of the development of therapeutic collaboration, taking as reference the concept of zone of Proximal Development. Based on this model and to analyze the therapeutic collaboration moment to moment, the authors developed the Therapeutic Collaboration Coding System - TCCS (Ribeiro et al., 2012). The main purpose of this present dissertation was comparing the development of therapeutic collaboration in a two cases, one of good outcome case and other of poor outcome case, followed within a narrative therapy individual modality, developed from the reauthoring model of White and Epston (1990). The poor outcome case held over 20 sessions and the good outcome case occurred in a total of 16 sessions. The sessions of both cases were coded based on the TCCS. The results concerning the evolution of therapeutic collaboration suggest that, in both cases, the therapist uses more challenging interventions. However, whereas in a poor outcome case, therapist supports the problematic experience more times, in a good outcome case the therapist support more often experience innovation client’s. With regard to the client’s responses, both tended to favor responses which indicated safety experiments. On the other hand, the client of the poor outcome case appeared more oscillating between tolerable risk and the intolerable risk throughout the process, and the client of a good outcome case showed a trend in their responses showing a decrease in experience of intolerable risk and increased response tolerable risk throughout therapy. The analysis performed for the State Space Grids (Lamey, Lewis & Granic, 2004) suggested that both the dyads of poor and good outcome case stabilized most of their therapeutic interactions in episodes whose client’s response indicated safety. These episodes, in turn, include different kinds of therapist´s intervention. The results will be discussed in view of the existing literature in this field of research and their contribution to understanding the therapeutic collaboration in a good and poor outcome cases, seeking to understand the influence of the therapy on the occurrence of interactions change these processes and raising hypotheses that can be tested in larger samples.por
